MidAmerica Hand to Shoulder Clinic will work with you to make sure you have all the necessary information and insurance coverage for your care. We ask that patients have their insurance information, state or military photo identification cards, completed forms and X-ray or other test results readily available when they arrive at our office. All copayments are due before the visit, when the patient checks in.

To make payments convenient for you, we accept cash, check and major credit cards such as Visa, Master Card and American Express.
